The following forms are available for use:

Borrower registration & PIN request. For staff currently employed in WA mental health. Please print this form, complete the details and send it in to the Library in order to register as a borrower and/or to electronically request photocopies from other libraries.

Student Borrower registration. For students currently based at Graylands Hospital. Please print this form, complete the details and send it in to the Library in order to register as a borrower.
